OPERATING LIABILITY AND INSTALLATION AND OPERATING INSTRUCTIONS.

PLEASE CAREFULLY READ THE FOLLOWING OPERATING LIABILITY AND INSTALLATION
AND OPERATING INSTRUCTIONS BEFORE USING THE SAKK® MAT. USE OF THE SAKK®
MAT INDICATES YOUR ACKNOWLEDGEMENT THAT YOU HAVE READ AND AGREED TO
THE TERMS OF THESE DOCUMENTS, UNDERSTAND THEM AND AGREE TO BE BOUND
BY THEIR TERMS AND CONDITIONS.

Liability Waiver

As a condition of purchase and before operating my SAKK® MAT Parapet Roof Cover,
I acknowledge that I have read my “Operating Liability” and “Installation and Operating
Instructions and I hereby, from the date of purchase, indemnify and hold harmless SAKK
Safety, LLC, its affiliates, officers and directors and any entity or individual associated with
SAKK Safety, LLC, from any liability, causes of actions, claims, demands, costs or debts of
any kind or nature, incurred or arising from or out of my or other’s use of the SAKK® MAT
Parapet Roof Cover, whether brought by me, my heirs, executors or assignees or by any
other third party.

Product Warranty

SAKK Safety, LLC warrants for a period of one (1) year from the date of purchase that the
SAKK® MAT is free from defects in workmanship, material, and design.

SAKK Safety, LLC warrants that all goods are of merchantable quality and are fit and
suitable for their purposes when used as stated in SAKK Safely, LLC Terms of Use and
Installation and Operating Instructions provided with the SAKK® MAT and must be
followed.

Operating Liability

I am aware that the use of the SAKK® MAT Parapet Roof Cover exposes me to hazards
that may result in injury and property damage. I understand that the SAKK® MAT
Parapet Roof Cover is capable of dangerous installation configurations and will be used
at heights that can cause injury to its user. I further understand that the user must be
vigilant to see that the SAKK® MAT PARAPET ROOF COVER IS PROPERLY INSTALLED
AND USED AT ALL TIMES IN ACCORDANCE WITH THE INSTALLATION AND OPERATING INSTRUCTIONS.

I further understand that it is my obligation to fully inspect the surface on which the SAKK®
MAT Parapet Roof Cover will be installed and operated, to ascertain that there are no
obstacles such as debris, rocks, loose roofing material, or other characteristics of the
terrain that may render the use of SAKK® MAT Parapet Roof Cover hazardous. This
includes inspection of surrounding areas, overhead obstacles such as tree limbs and power
lines and the like.
